Vision Assistance Program The purpose of the Vision Assistance Program is to help individuals within the Texas Lions District 2-E2 who are in the country legally with a one time monetary assistance to provide vision care that cannot be funded by regular means. Vision Assistance Protocol: 1. Committee made aware of possible need for assistance. 2. Application for assistance filed to inform the committee. 3. Committee questions clarified via email or meeting. 4. Determination of whom (hopefully a Lion's member or agent familiar with the Lions Programs) will be responsible for guiding the candidate through his/her medical journey. This guide will be the fiduciary guard of all donated funds from the Vision Assistance Committee. Checks must be written to Institutions/Medical Providers, not to individuals. 5. Committee votes on donation if funds are available. 6. Guide is provided a list of possible agencies/doctors/institutions that should be contacted about aid and/or reduced fees. 7. Guide follows up with the Vision Assistance Committee on the progress of the patient and returns all unused funds on or before the one year anniversary of the request. Donations: The Vision Assistance Program was begun with the generous donations of the Lions Clubs and the program will gladly accept charitable donations and grants as the program does exist as a part of the Lions Organ & Eye Bank, which is a 501(c)(3) charitable organization. All contributions should be made payable to the LOEB Vision Assistance Fund and mailed to: |
